Panel: Hospital to home: Tomorrow’s transitional care models (Health Journalism 2011)
Speaker: Mary Naylor, Ph.D., F.A.A.N., R.N., Marian S. Ware professor in Gerontology, University of Pennsylvania School of Nursing
Panel description: Among other things, the Affordable Care Act mandates hospital hand-offs – “transitions” in policy speak – that reduce readmissions, make better use of community-based health care resources and lower health care costs. This session explains the key players in transitional care, the services they are expected to provide, the challenges they face because of the high degree of integration all transitional care models require, and the huge role family caregivers are expected to play.
