On the 10th anniversary of IOM's landmark study "To Err is Human," Consumers Union will focus on the realization of as-yet unachieved patient safety goals. Speakers will include AHCJ president and ProPublica senior reporter Charles Ornstein and former U.S. Treasury Secretary, Paul O’Neill.
