Story showed that a hospital that had a patient die when he was accidentally overdosed by hospital staff continued to have problems with overdoses.

I used hospital inspection reports. No FOIA was required.

There haven’t been any reports of overdoses since the story was published.

Hospital inspection reports in Pennsylvania are available online. There is no shortcut to finding which ones have important information. You have to make reading them one-by-one a weekly priority.
