Improving community health takes much more than hospital-sponsored educational sessions and free literature. Today's hospitals are forming innovative partnerships with community groups to bring health care into homes and neighborhoods. They are finding that approaching intractable problems in new ways, such as by providing transportation or increasing access to healthy food, can make a big difference.

This article used data and information from the American Health Planning, the Association for Community Health Improvement, the Center for Health Improvement and the National Association for Community Health Centers.

Sources included experts from he American Health Planning, the Association for Community Health Improvement, the Center for Health Improvement and the National Association for Community Health Centers, hospital employees and trustees, and community group leaders.
