These are benefits state or federal laws require of all health insurance policies to provide to insured individuals.

The Marketplace at healthcare.gov

In 30 states, the federal Department of Health and Human Services runs the Marketplace for consumers enrolling in health insurance under the Affordable Care Act as of mid-2023. There are 17 states and the District of Columbia operating state-based marketplaces (SBMs) and three states that run their marketplace using the federal marketplace system. The Marketplace and the SBMs are the only sites where consumers can buy comprehensive health insurance that fully complies with the requirements of the ACA. (See also: state-based marketplaces.)
