The New Jewish Home’s 14th annual Palliative Care Conference, Being Comfortable with Life and Death: Effective Models of Advance Care Planning and Medical Orders, will explore the most recent models in training clinicians in and reimbursement for end of life conversations. With an estimated 50% of Americans over 85 suffering from dementia, elderly patients’ ability to make decisions has become one of the most important issues in geriatric care today.
Christine Wilkins, advanced care planning program manager at NYU Langone Medical Center, will present “Investing in End-of-Life Conversation Planning”; Pat Bomba, medical director at Geriatrics Excellus BlueCross BlueShield, will discuss how to implement NYS’s eMOLST (Electronic Medical Orders for Life-Sustaining Treatment) system; and Penny Osmon Bahr, a director at Avastone Health Solutions, will offer “CMS and Reimbursement for End-of-Life Conversations.”
There will be several breakout sessions including a deeper dive into implementing eMolst, and the implementation of the Gunderson Project method of end of life conversations. Speakers featured are Pat Bomba, and Sally Kaplan, Director, What Matters: Caring Conversations about End of Life.
