There are 10 virtual workgroups which are working toward how to improve the capture and continuing use of nurse-sensitive data using national standards to to achieve the quadruple aim of better health\, better patient experience\, lower costs and improved work life of health care providers. \nKeynote: Longitudinal\, Patient-Focused\, Interdisciplinary Care\nRebecca Freeman\, Ph.D.\, R.N.\, P.M.P.\, of the ONC Chief Nursing Officer of the Office of the National Coordinator for Health Information Technology\, U.S. Department of Health and Human Services\nDelivery system reform will change the way health care is provided and paid for in the United States. Reimbursement will be based on patient outcomes across the care continuum; optimal outcomes will be achieved through efficient\, team-based care that is expertly coordinated across care settings. Ideally\, the movement of information from one care environment to the next must occur. Nurses are natural care coordinators and an integral part of any patient’s health optimization\, but nursing documentation in some care settings is not always sharable and comparable with that of the rest of the care team. DESCRIPTION:The U.S. Conference of Mayors and National Alliance to End Homelessness to Release 2016 Hunger and Homelessness Survey \nThe U.S. Conference of Mayors (USCM) will release its Annual 2016 Hunger and Homelessness Report on Dec. 14 at 11 am (EST) featuring results from 39 cities from across the country. \nThis year\, the Conference is working in conjunction with the National Alliance to End Homelessness\, a leading voice on homelessness research in the nation. Using the Alliance’s expertise\, this year’s mayors report is able\, for the first time\, to present a snapshot of homelessness in U.S. cities as well as compare trends in homelessness between cities and the nation as a whole. \nThe report includes results from a broad array of America’s major urban centers (listed below)\, and underscores the fact that despite the economic recovery\, the majority of study cities in 2016 had rates of homelessness higher than the national rate of homelessness.
